Alcoholic Beverage License Holders should use this form when they want to employ a minor to sell or serve alcohol. This permit is not required for minors employed to perform duties other than selling or serving alcohol (e.g. hostess, busser, etc).The license holder (NOT the minor) must complete and submit this form and the minor’s Department of Labor issued work permit in order for this application to be processed.

Use this form to request permission from the State’s Attorney to remove human remains from a burial site.  Funeral Home requestors must also provide a Disinterment Agreement executed by the decedent’s next of kin.
